We offer access to a variety of financial resources to make health care affordable to everyone. We accept Medicare, Medicaid, all private insurance plans, and offer a Sliding Fee Scale program for patients who are uninsured or underinsured.
Sliding Fee Discount Program
In order to prove your income and qualify for the sliding fee discount program, please bring any and all of the following documents that may apply to you:
- Pay check stub
- SC Employment Security Commission Report
- TANF
- Retirement Statement
- Social Security Statement
- Social Security Request
- Housing Authority Request
- Child Support/Alimony Report
- Federal/State Tax Return
- Prior Year W-2/1099
- Medicaid Application/Statement
